To protect your consumer rights, we need information from you verifying your identity before we can process your request. Please either fax or mail us a letter with:
- Your full name
- Current address
- Previous Address
- Phone number
- Social Security Number (SSN)
- Date of birth (DOB)
- Clear copy of your Driver’s License or other form of state issued I.D. Please ensure the copy is legible and can be read easily.
